Consent Form (Coached Activities)
If you've booked onto one of the sessions below, you'll need to submit a Medical Consent form on the day of your session.
Please note, if you're under 18 or, the form will need to be countersigned by a parent or guardian, and depending on the size of your group you may need an adult to accompany you on the day. It is compulsory for all unsupervised paddlers to have an ID card to use our Park & Play facility at the centre.
There is an annual £5 charge for this card, which is available to buy from reception.
The good news is that this means that you only have to fill out one Park & Play disclaimer and it'll cover you for a full year!
This card will be used as a deposit against the bibs that we issue on Park & Play sessions. If you don't return your bib, you'll be charged for another card. Cards will be kept at reception while you are paddling and handed back to you on the successful return of your bib! If you forget your card, you'll be charged another £5 for a new card - so please keep it with you!
